NHS England showcases IPC integration support

A project by IPC working with Haringey CCG was selected by NHS England as a case study to showcase how local areas are delivering integrated care and support.   The project was also showcased at the Whittington Health Education Conference in March 2016.

IPC supported Haringey Council and CCG over 18 months to deliver integrated practice through Locality Team working and improve the coordination of care to Haringey’s vulnerable older people. 

This involved the design and delivery of:

  • Listening Events for more than 100 frontline staff to explore good practice, frustrations and blockages and give them opportunities to think creatively about how they can work together in themed workshops to prepare staff for integrated Locality Team working. Themes included Skills for Leading Change, Understanding Professional Roles and Joint Assessment & Care Planning;
  • Facilitated action learning support for Team Managers to work on real problems facing them in making integrated care a reality and to encourage reflective learning
  • More recently, IPC have introduced the role of Integrated Change Champion across health and social care services to help embed the vision and build local capacity for integrated working. This has involved identifying the qualities, skills and behaviours needed to facilitate the shift in culture needed for successful change.
