New discussion paper; reducing delays in transfers of care

A new paper from Professor John Bolton discussing how local authority and health commissioners of out of hospital care services for older people determine the amount of care that is required to meet local demand.  Based on work undertaken in England, Scotland and Wales and with reference to other published reports, Professor Bolton has structured his considerations using a basic commissioning cycle – analyse, plan, do and review.  This approach reinforces the discipline of evidence-informed approach, and the importance of good performance management approaches.
