Helping the CoPs with their enquiries….IPC and the Welsh Government Regional Integration Fund

The Welsh Government’s first joint plan for health and social care, ‘A Healthier Wales: Our Plan for Health and Social Care,’ presented a vision and plan for the future of health and social care in Wales. This included a Regional Integration Fund to support local and regional innovation.

As part of this programme IPC are facilitating 6 long-term Communities of Practice (CoPs) to share best practice in areas such as:

  • community-based care
  • emotional and mental health
  • hospital to home provision.

Significant progress has been made in the last year and a summary newsletter in Welsh and English can be downloaded below.

Emeritus Professor Keith Moultrie says, “These Communities of Practice are great to work with. They are networks of practitioners from across Wales sharing approaches that work and they are helping each other with advice and practical support. They are a key part of the drive for integrated models of care in Wales.”
